Social Services Unit

The Social Services Unit (SSU) provides personal support and social services to Adult Services participants at Hudson Guild’s Fulton Center. This includes intensive and walk-in case management for both homebound and mobile seniors, information and referral services, and access to additional services for legal, medical and housing needs. After going through a streamlined intake process which allows for gathering pertinent information in one sitting, each senior is assigned to a case manager or case aide who maintains primary contact with the senior and is responsible for assessing needs, coordinating services, and monitoring results.
